The institution began as a district hospital in Kadapa City, later upgraded to a tertiary care teaching hospital. It was formally established as Rajiv Gandhi Institute of Medical Sciences and inaugurated on 27 September 2006 by UPA Chairperson Smt. Sonia Gandhi.
On 1 August 2006, the college began its first academic session with MBBS students. Over the years, it expanded intake to 175 MBBS seats and introduced postgraduate training across multiple specialties.
In December 2023, former Chief Minister YS Jagan Mohan Reddy inaugurated the YSR Cancer Hospital Block, Superspeciality Hospital, and Psychiatry Block, making it one of the most advanced government-run teaching hospitals in Andhra Pradesh.
